    NGC (1999-2008) state quarter deep obverse die cap.
    The coin stuck to the obverse die when it was struck on top of another planchet, and then proceeded to be struck into incoming planchets, causing the coin to "wrap" around the die's neck. A dramatic major state quarter error.
